Communications and frameworks

  • Archipel newsletters

    Our newsletters are intended for our health and community services partners, as well as the general public, including patients and families. We provide updates on our most recent activities and those of our partners.

  • Declaration of values

    At Archipel OHT, the Patient, Family and Caregiver Declaration of Values for Ontario is central to our work, guiding our commitment to patient-centered care, dignity, and respect. This Declaration emphasizes the importance of involving patients, families, and caregivers in healthcare decisions, ensuring their voices are heard and respected. 


    As part of our Patient Family and Caregiver Advisory Committee (PFCAC) mandate, we actively align with these values to promote a more inclusive and equitable healthcare experience. For more information, view the Declaration of Values.


  • Equity, Inclusion, Diversity and Anti-Racism

    The Equity, Inclusion, Diversity, and Anti-Racism (EDIA) Framework of Ontario Health is a commitment to identify and address anti-Indigenous and anti-Black racism in the health care system. This essential tool guides work to build an organizational culture focused on equity, inclusion, diversity, and anti-racism, and to contribute to better outcomes for patients, families, and providers within the health care system.   • Ontario Health Teams

    The province is building a connected health care system through the creation of Ontario Health Teams. To follow the system’s developments, visit Ontario Health Teams.



Regional programs and services

The following programs aim to improve access to care for patients. They were developed through multisectoral collaboration between several Archipel partners and the involvement of client partners as community representatives.

  • Access CSS

    Community Support Services support seniors and adults with disabilities. The goal is to support individuals to remain safe, independent and healthy at home. Community Support services are funded by Ontario Health East. These services play a vital role our health care system.


  • Access MHA

    Access MHA provides a coordinated regional access to services for people with mental health and addiction issues in the Champlain region. Providers can also refer clients and patients.


  • East Region Virtual Care Clinic

    The East Region Virtual Care Clinic (ERVCC) provides Ontario Health’s East Region patients with timely access to care through virtual appointments. The ERVCC is open from 1:00pm - 9:00pm, seven days a week.


  • Archipel navigation and referral network

    Launched in December 2023, the network allows community care and service providers to use the Caredove platform to display their services and accept appointment requests from clients and partners. 

    • The Caredove platform integrates well with most other applications and systems used in the health and community services sector. To learn more, check out an infographic on navigation, or visit the Caredove website.

  • Post-Surgical Remote Monitoring Program

    Launched in April 2022, the program aims to improve access to appropriate care by transitioning patients from hospital to home. It also reduces readmission rates after surgery and the number of visits to the emergency room. 


    This collaboration between Montfort Hospital, Hawkesbury and District General Hospital, Ottawa Paramedic Services, and the Prescott-Russell Community Paramedics Services is becoming an alternative to hospitalization. The program also improves patients and families’ experience and satisfaction.

  • Black health

    Training: How to Provide AntiRacist Mental Health Care

    The training How to Provide AntiRacist Mental Health Care is available free online and is accredited by the Canadian Psychological Association. The training program includes six modules. A certificate is awarded to those who complete the pre- and post-training forms. For more information, visit the course page.

     

    The Ottawa Black Mental Health Coalition 

    The Ottawa Black Mental Health Coalition (OBMHC) works collaboratively and collectively to improve mental health outcomes for Black communities in Ottawa. The Coalition is hosted by Somerset West Community Health Centre (SWCHC), which provides the Coalition with administrative and supervisory support.

     

    Their goal is to achieve a more equitable mental health system by improving access to culturally competent mental health support and resources, offering healthier outcomes Black residents in Ottawa. For more information, visit Ottawa Black Mental Health Coalition.


  • French-language health services

  • Indigenous health

    Wabano Centre

    Wabano is a cultural, health, and excellence centre by and for Indigenous people. Located in Ottawa, the Wabano Centre offers programs, services, activities, and support for all age groups. For more information, visit Wabano Centre.


    The Wabano Centre for Aboriginal Health and the Ottawa Aboriginal Coalition have jointly produced a report on Indigenous-specific racism and discrimination in health care across the Champain region. The report highlights anti-Indigenous experiences and eye-witness accounts. It also makes several recommendations to end racism and discrimination, and to improve health equity in Ontario's healthcare system.  For the full report, visit Share Your Story.



    Indigenous Grassroots Circle

    The Indigenous Grassroots Circle (IGC) organization unites Métis, First Nations, Inuit and non-indigenous (allies) volunteers throughout Eastern Ontario. The group creates Indigenous led and themed events as well as the development of community and economic projects that will benefit the entire community. IGC aims to fill important social and cultural gaps. For more details, visit Indigenous Grassroots Circle.


  • Support for caregivers

    Caregivers provide physical and emotional support to a family member, partner, friend or neighbour. But as a caregiver, they also need support, information and respite. The following websites offer resources for caregivers.

  • Support for the Jewish community

    People of all faiths have the right to live in safety and security, free from intimidation and violence. Archipel produced a list of emergency resources and support programs for the Jewish community and its allies as well as information about combatting antisemitism.
