We are a coordinated group of health and community service organizations and professionals based in Ottawa East and the United Counties of Prescott-Russell providing health care and services in both official languages.
We work together to create a network in which people have access to the right care, the right team and the right setting, at the right time.
Our mission is to coordinate multi-sectoral collaboration and co-design with clients to optimize equitable access to a continuum of care and services that meet the needs of our population in both official languages.
Our shared vision is to build a trusted community network that is diverse, inclusive and equitable, able to respond to the varied and evolving needs of its population, so that each individual can reach their full potential for health and well-being throughout their lives.
Our Ontario Health Team (OHT) was born in late 2019, through the collective work of 25 partners. Our team continues to grow and now includes 60 partners as our territory now extends to the United Counties of Prescott-Russell.

We have several committees and groups working on integrated care projects in Ottawa East and Prescott-Russell.
Ottawa East includes the Vanier Ward, Lowertown and Orléans. Prescott-Russell includes eight municipalities: Clarence-Rockland, Hawkesbury, Casselman, Alfred and Plantagenet, Champlain, Hawkesbury East, The Nation and Russell.
