Partners in your health.
At the heart of your community.
Archipel Ontario Health Team is a group of organizations and individuals offering health care and community services in both official languages. We serve the eastern part of Ottawa and the United Counties of Prescott-Russell, which includes 200,000 people.
Archipel partners work together to create a network in which people have access to the right care, in the right setting, at the right time.
An OHT includes hospitals, medical clinics, physicians, nurse practitioners, physiotherapists and many other healthcare professionals. Homecare, community and social service organizations are also part of an Ontario Health Team. All these partners aim to work together, as one large coordinated team within a region. Ottawa East includes the Vanier Ward, Lowertown and Orléans. Prescott-Russell includes eight municipalities: Clarence-Rockland, Hawkesbury, Casselman, Alfred and Plantagenet, Champlain, Hawkesbury East, The Nation and Russell.
Coordinate multi-sectoral collaboration and co-design with clients to optimize equitable access to a continuum of care and services that meet the needs of our population in both official languages.
A trusted community network that is diverse, inclusive and equitable, able to respond to the varied and evolving needs of its population, so that each individual can reach their full potential for health and well-being throughout their lives.
