You can help us create a better system focused on the needs of patients, families and clients by:

Survey for patients and caregivers affected by respiratory problems

This survey is for people living with breathing problems, persistent coughing or frequent respiratory infections.


The goal of the survey is to better understand how you use existing resources to manage your health condition, and how we can improve their access to make life easier for you and your loved ones.


How to answer the survey?

You can use your computer, your tablet or your smartphone to answer the online survey.

  • The survey is open February 3-28, 2025 and should take 20-30 minutes to complete.
  • The first 100 people to complete the survey will receive an invitation to accept a thank-you gift card.
  • You can answer the survey in English or French. By clicking on the link, you will open the online survey.

Archipel Ontario Health Team (OHT) is a coordinated group of organizations and individuals offering health care and community services in both official languages. Archipel partners work together to create a network where people have access to the right care, in the right place, at the right time.

Participate in Archipel’s Patient, Client and Family Advisory Committee


The mandate of this committee is to contribute to the development of Archipel’s programs, services and initiatives. The goal is to improve the care provided, as well as the experience and quality of life of our populations. The committee also plays an active role in patient, client and family engagement initiatives, and contributes to Archipel’s strategic planning.


The inclusion of patients, clients and their families allow us to benefit from their diverse expertise and lived experiences. Would you like to join this committee or find out more about their activities?

We have working groups focused on priority populations and quality improvement projects. These groups need the expertise of people with direct experience of the services and systems that impact these projects and populations. Would you like to join our working groups or find out more?

Take a survey 


We regularly survey our target populations to learn more about their needs. If a survey is in progress, we’ll post instructions on how to complete it here.
