The Archipel Ontario Health Team (OHT) works with service providers to improve access to care for all people living in Ottawa East and the United Counties of Prescott-Russell.
The partners of the Archipel Ontario Health Team (OHT) work together to build a diverse, inclusive and equitable community network that can meet the varied and evolving needs of its population. Our OHT aims at ensuring that each individual can reach their full potential for health and well-being throughout life.
Our OHT works collaboratively with the stakeholders and users of the health system to achieve a common goal: a more equitable and health system redesigned by and for its population. Multisector collaboration and the co-design with client-partners are therefore aimed at planning and providing equitable access to an integrated and coordinated continuum of care and services.
To move forward, we need to make decisions—and we make them collaboratively. In order to work in a concerted and efficient manner, the members of the Archipel OHT sign and commit the Collaborative Decision-Making Agreement (CDMA) and Share-Leadership Framework (PDF).
We value diverse partnerships and shared leadership to promote the best use of limited resources for the common good and to achieve our collective vision.
The Archipel OHT members are the representatives of the organizations that signed the CDMA, as well as the community representatives (partner clients). Our team includes close to 70 members. Archipel has two categories of members: Partner and Affiliate. Check out the list of our members.
A partner is an entity thoroughly engaged in Archipel OHT initiatives and ultimately accountable for the success and achievement of the shared objective. Signature of the CDMA is required to obtain and maintain this status.
An affiliate member is a subject matter expert; or an agency that spans multiple OHTs; or who is not predominantly funded by the Ontario Ministry of Health and the Ministry of Long-Term Care. Affiliates have interests that are aligned with the OHT’s shared objective.
Membership in our Ontario Health Team is voluntary. Applicant organizations must pursue the shared objective and commit to the collective vision, values and guiding principles of the CDMA. The Executive Committee is responsible for evaluating applications and accepting new members.
