FAQ for patients and families
Frequently asked questions
Archipel Ontario Health Team (OHT) was officially formed in April 2023. We work with service providers to improve access to care for everyone living in Eastern Ottawa and the United Counties of Prescott-Russell.
Our OHT includes hospitals, medical clinics, family physicians, nurse practitioners, physiotherapists and many other health and community service professionals. Patients and families also play an important role by participating as patient partners or committee members.
I do not have a family doctor. Can you help me find care for myself and my family?
We are not a healthcare provider, but we have created a list of resources to help you find a family physician, nurse practitioner or family health team.
Please see the Unattached patients, Resources for patients and families or Find services pages.
Has Archipel brought about changes in the way healthcare is delivered in the region? If so, what are they?
In collaboration with our partners, we have undertaken or facilitated several projects to improve access to care for patients, clients and families. Here are some of the projects we have worked on.
• Online appointment booking system - In June 2023, we launched a project to facilitate the implementation of an online appointment booking system in which seven medical clinics participated. To learn more about the improvements offered by this system, read the article Online appointment booking: a win-win solution.
• Memory reconsolidation therapy - Since September 2023, this therapy has been offered to patients in Montfort's Emergency Department who have symptoms of post-traumatic stress disorder (PTSD) or are at risk of being affected by it. This project is an initiative of our Mental Health and Addiction Working Group. To learn how this service helps people, read the article Memory Reconsolidation Therapy: changing your life after post-traumatic stress disorder.
• Postnatal Wellness Clinic – Launched in March 2022, this clinic provides rapid access to care for patients who have recently given birth at Montfort and who show signs and symptoms of postpartum depression or anxiety but do not have a primary health care provider. To learn more about how the clinic works and how one patient benefited from it, read the article Grateful mother says everyone should have access to Archipel Postpartum Wellness Clinic.
• Archipel Navigation and Referral Network – In December 2023, we launched an online tool that allows patients, clients and families to search for the health care and community services they need, close to home. The network also offers the option of booking appointments online with registered providers. The network can be found on the page Find Services.
• Post-Surgical Remote Monitoring Program – Launched in April 2022, this program aims to improve the transition from hospital to home after surgery and enhance the experience and satisfaction of patients and families. To learn how this program helped one patient, read the article Rockland couple very satisfied with Post-Surgical Remote Monitoring Program.
Do I need to register or do anything to benefit from Archipel programmes?
You do not need to register or do anything to benefit from our programmes. Archipel does not have patients or clients, but rather works in collaboration with care and service providers. As a client or patient, you can access our programmes through your doctor, hospital, medical clinic or other health professionals.
I would like to help improve our healthcare system. Is it possible?
Absolutely! Patients, clients and families are at the heart of our service planning and decision-making. You can help us create a better healthcare system by joining one of our committees or working groups, responding to surveys or participating in discussion forums.
To better represent the diversity of our population, we are seeking members to serve on the Patient, Client and Family Advisory Committee.
